Creating and sharing forms
Create reusable Forms, share them with a public link, and review responses in Introwise.
Forms let you create reusable questions, collect answers during booking, or share a standalone link. Forms are included in Growth and Pro plans.
Forms are currently rolling out in private beta. If you are on an eligible plan and do not see Forms in your dashboard, contact support.
Create a form
- Go to Dashboard > Scheduling > Forms.
- Select New form.
- Enter a title and, optionally, a description that explains why you are asking the questions.
- Choose whether responses submitted through the public link must include the client's name and email. If you leave this off, public-link responses are anonymous.
- Add up to five questions. Each question can use a short-text or long-text answer, include optional help text, and be required or optional.
- Select Create form.
Share a form with a public link
Every form has a standalone link that uses your Introwise booking-page experience. You can use it for intake, feedback, testimonials, or any other questions that do not need to be part of a booking.
- Open Dashboard > Scheduling > Forms and select the form.
- Copy the URL in Link to this form.
- Send the link to clients or add it to your website, email, or community.
Review responses
- Open the form from Dashboard > Scheduling > Forms to see all responses to that form.
- Open a client record and select Forms to see identified responses connected to that client.
- When a form is completed during booking, the answers also appear on the related session.
Reuse and edit forms
The same form can be shared with its public link and attached to one or more services. If you edit a form used by multiple services, the updated questions apply to future bookings on all of them. Existing responses keep the questions and answers that were submitted at the time.

Troubleshooting
Forms is not shown in the dashboard
Confirm that your account is on Growth or Pro. Forms are still rolling out in private beta, so contact support if the menu is missing from an eligible account.
A public form link says the form is not available
Check that you copied the complete link from the form's editing page. If the link still does not work, contact the expert who shared it or Introwise support.
